Healthcare
A community health center needs to increase rates of diabetic eye exams among its patients to meet its quality metrics, earn revenue from Medicaid, and close health equity gaps among its population.
commonflower facilitates a quality improvement process to convene an action team, understand root causes through stakeholder interviews, literature review, and data analysis, develop an intervention and evaluation plan, and provide project management support to reduce the disparity.

Community Coalition
Community Health Worker programs in the New England states made great progress during the pandemic response, but have met a funding cliff and a backlash against health equity. CHW employers need technical assistance and funding to sustain programs. CHWs need professional and leadership development. And stakeholder groups need to coordinate.
commonflower facilitates establishment of a regional coalition of CHW employers and allies to strategize and pool technical assistance resources across state lines.

A foundation wants to catalyze partnerships between healthcare and community organizations to address social needs, addressing the roots of health outcomes. But there are many contracting and operational models, they have costs, and there are concerns about power-sharing and fit.
